Maybe throttling at the source would help.  If Postfix launches a
process for each arriving mail and /then/ applies in_flow_delay if
necessary, the machine may be getting choked with Postfix processes.

Do you see a spike in process count and load average?

I used to send dozens messages from a shell "for" loop, and I would
have to stick in a brief "sleep" after each mail to prevent choking.
But this was with qmail and then exim.  Postfix may be smarter.
