I run a MySQL system as the backend for the USGS Earthquake
Notification System:

We have about 100,000 people signed up for this. The database
stores their notification rules and preferences. When an 
earthquake comes through, I query the database to get a list
of people who are interested in hearing about it. It was  a
fun little project to implement.
